(UTDD) loại ung thư phổ biến thứ giới với 1.033.000 trường hợp mắc năm [1] Hàn Quốc nơi có tỷ lệ mắc cao Tại Việt Nam, 14,5% trường hợp mắc ung thư UTDD Đây nguyên nhân gây tử vong thứ hai giới (sau ung thư gan phổi) [2] Dù có tiến chẩn đốn điều trị UTDD bệnh lý quan trọng cần tập trung nghiên cứu Có nhiều giả thiết nguyên nhân dẫn đến UTDD trực khuẩn Helicobacter Pylori (gặp 90% trường hợp) yếu tố nguy quan trọng [3] Các yếu tố nguy khác kể đến tiền sử bệnh lý dày, đồ ăn hun khói, hút thuốc lá…[4, 5] Khoảng 10% ung thư biểu mơ dày có biểu yếu tố gia đình, nhiên – 3% gây hội chứng ung thư dày di truyền Bao gồm: ung thư dày lan tỏa di truyền (HDGC), polyp tuyến gia đình, ung thư đại trực tràng khơng polyp di truyền, hội chứng polyp người trẻ tuổi HDGC chẩn đoán theo hướng dẫn đồng thuận Hiệp hội liên kết Ung thư dày giới (International Gastric Cancer Linkage Consortium IGCLC) [6] Đến nay, đột biến quan trọng xác định bệnh HDGC đột biến gen CDH1, phát 30-40% số trường hợp Đột biến dòng mầm CDH1 phát Guilford thơng qua phân tích đột biến gia đình người Maori (New Zealand) mắc UTDD lan tỏa khởi phát sớm [7] Gen quy định tổng hợp E – cadherin, protein đóng vai trò quan trọng việc bám dính liên kết tế bào Khi xảy đột biến gen CDH1, mức độ biểu E-cadherin giảm đi, từ giảm độ liên kết tế bào,dẫn đến di tế bào ung thư Việc xác định đột biến gen CDH1 xác định gia đình có nguy cao mắc hội chứng HDGC quan trọng việc quản lý, theo dõi, tiên lượng điều trị dự phòng HDGC Tuy vấn đề thời có ý nghĩa lớn, nhiên Việt Nam, HDGC nhìn chung chưa quan tâm mức lâm sàng Những cá nhân gia đình có nguy cao mắc HDGC thường khơng quan tâm chẩn đốn tư vấn đầy đủ Ngồi ra, chưa có nhiều nghiên cứu UTDD di truyền đột biến gen CDH1 Những điều khiến định làm tiểu luận “Đột biến gen CDH1 bệnh nhân ung thư dày lan tỏa di truyền” để giúp người có góc nhìn đầy đủ vấn đề sau phục vụ cho đề tài “Nghiên cứu đột biến gen CDH1 bệnh nhân ung thư dày lan tỏa di truyền” SƠ LƯỢC VỀ UNG THƯ DẠ DÀY 1.1 Dịch tễ học ung thư dày Theo GLOBOCAN 2018, UTDD đứng hàng thứ số loại ung thư phổ biến, với khoảng 1.033.000 ca mắc năm giới (chiếm 5,7%) Theo ước tính, năm có 783.000 người tử vong UTDD, đứng thứ tất loại ung thư [1] Hàn Quốc nước có tỷ lệ mắc cao Ngược lại, Bắc Mĩ, châu Phi Bắc Âu nơi có tỷ lệ mắc thấp [8] Tỷ lệ mắc chuẩn theo tuổi nam giới cao gấp lần nữ giới, cụ thể nam giới 15,7/100.000 nữ giới 7,0/100.000 [1] Việt Nam nằm khu vực có tỷ lệ mắc UTDD cao Theo Đoàn Hữu Nghị, tỷ lệ mắc UTDD nam giới 19,3/100.000 người số nữ giới 9,1/100.000 người [9] UTDD nguyên nhân gây tử vong đứng hàng thứ hai giới [2] 1.2 Các yếu tố nguy ung thư dày Có nhiều yếu tố gây tăng nguy mắc UTDD Có thể chia yếu tố nguy thành nhóm: mơi trường biến đổi di truyền 1.2.1 Môi trường, lối sống Các yếu tố môi trường, lối sống chứng minh liên quan tới UTDD lượng muối tiêu thụ, thức ăn nhiều nitrate loại cá, thịt chế biến sẵn, loại thức ăn xơng khói, hút thuốc [10-12] Ngược lại, ăn nhiều rau xanh, trái cây, vitamin C làm giảm nguy mắc UTDD Những chứng nhấn mạnh tầm quan trọng chế độ ăn uống UTDD 1.2.2 Trực khuẩn Helicobacter Pylori (H.P) Trực khuẩn H.P yếu tố nguy quan trọng gây UTDD, gặp 90% bệnh nhân UTDD [3] Bất thường H.P gây tổn thương niêm mạc, viêm niêm mạc dày sau dị sản, loạn sản ung thư Tuy nhiên, có điều thú vị châu Phi, tỷ lệ UTDD tương đối thấp tỷ lệ nhiễm H.P lại cao [13] Nghiên cứu sau tìm đồng 10 nhiễm giun sán H.P, hệ miễn dịch thay đổi cách đáp ứng từ tế bào T helper sang T helper dạng gây tổn thương hơn, góp phần giải thích cho tượng [14] Hút thuốc làm tăng tác dụng gây ung thư vi khuẩn H.P [15] 1.2.3 Yếu tố di truyền Hiện nay, tỷ lệ mắc UTDD bệnh nhân lớn tuổi có xu hướng giảm dần Tuy tỷ lệ mắc bệnh người trẻ hay ung thư dày có tiền sử gia đình giữ mức ổn định Các nhà khoa học xác định yếu tố di truyền đóng vai trò quan trọng UTDD với khoảng 10% số bệnh nhân [16] Nhiều gen gây UTDD phát Trong gen thường gặp bệnh nhân UTDD CDH1, chiếm 25-30% bệnh nhân UTDD có tính chất di truyền [17] 1.3 Triệu chứng ung thư dày Các triệu chứng bệnh UTDD giai đoạn sớm thường khơng có khơng đặc hiệu, nhầm lẫn với triệu chứng bệnh lành tính khác Các triệu chứng hay gặp giai đoạn sớm UTDD đau bụng vùng thượng vị, ợ hơi, khó tiêu, chán ăn, buồn nơn nơn Các triệu chứng tồn thân gặp giảm cân khơng rõ ngun nhân, mệt mỏi, có khơng có thiếu máu Khi bệnh giai đoạn muộn xuất triệu chứng, biến chứng suy kiệt, hẹp mơn vị, xuất huyết tiêu hóa, thủng dày hay biểu di hạch thượng đòn trái, cổ chướng 1.4 Chẩn đốn ung thư dày Nội soi kết hợp bấm sinh thiết vùng tổn thương nghi ngờ làm mô bệnh học coi tiêu chuẩn vàng chẩn đoán ung thư dày, đồng thời tránh bỏ sót ung thư sớm Các bất thường phát nội soi viêm, vết lt, hình ảnh tổn thương ung thư ổ xuất huyết… Chẩn đoán giai đoạn UTDD dựa chủ yếu vào cắt lớp vi tính siêu âm nội soi Việc xác định xâm nhập ung thư vào thành dày lan 47 2440-6C>G 2494G>A (V832M) chr16:del 6742429867425126 (828 bp) Intron 15 Exon 16 ĐB vị trí nối ĐB sai nghĩa-Gây bệnh [100] Exon 16 Mất đoạn lớn [73] Điểm yếu chung nghiên cứu bệnh số lượng bệnh nhân ít, gây khó khăn thu thập mẫu tiến hành nghiên cứu có giá trị độ tin cậy cao Phần lớn công bố áp dụng phương pháp PCR truyền thống direct sequencing nhằm tìm bất thường gen CDH1 Việc áp dụng phương pháp NGS nhằm tìm đột biến gen áp dụng chi phí cao Tuy tương lai, phương pháp tân tiến cần áp dụng nhằm tìm bất thường nâng cao hiểu biết bệnh 3.9 Nghiên cứu đột biến gen CDH1 Việt Nam Tại Việt Nam chưa có nhiều cơng trình nghiên cứu vấn đề Theo tìm hiểu tác giả, có đề tài “Nghiên cứu đột biến số vùng trọng điểm mức độ methyl hóa gen CDH1 bệnh nhân ung thư dày lan tỏa di truyền” tác giả Ngô Diệu Hoa nghiên cứu bệnh Nghiên cứu thực 16 bệnh nhân đáp ứng tiêu chuẩn chẩn đoán HDGC hội IGCLC, đánh giá đột biến gen CDH1 tập trung exon 15 đánh giá độ methyl hóa vùng promoter CDH1 Theo kết nghiên cứu xác định bệnh nhân (chiếm 6,25%) có đột biến gen CDH1 exon 09 đột biến chưa công bố 48 KẾT LUẬN Ung thư dày ung thư phổ biến thứ giới, với 923000 trường hợp mắc 723000 trường hợp tử vong năm Tuy tỷ lệ mắc UTDD giảm dần, vấn đề quan tâm hàng đầu Trong UTDD yếu tố di truyền, cụ thể UTDD lan tỏa di truyền (HDGC) ý nhiều tính chất phức tạp Tuổi khởi phát bệnh trung bình 38 tuổi (dao động từ 14-69 tuổi) với nguy mắc UTDD tích lũy 70% nam 56% nữ đến 80 tuổi Những bệnh nhân có UTDD cần đánh giá theo hướng dẫn đồng thuận năm 1999 2010 tổ chức IGCLC để sàng lọc Sau đó, đối tượng đáp ứng tiêu chuẩn đánh giá nhằm phát đột biến gen CDH1 kĩ thuật sinh học phân tử, chủ yếu phương pháp PCR thơng thường kết hợp giải trình tự gen MLPA Thơng thường, khoảng 30-35% đối tượng tìm thấy đột biến gen CDH1 Với trường hợp lại, nghiên cứu chưa tìm đột biến gây bệnh Một số phương pháp pyrosequencing, next generation sequencing… nghiên cứu áp dụng để trả lời cho câu hỏi UTDD lan tỏa di truyền bệnh tương đối Hiện tại, chưa có nghiên cứu đánh giá điều trị riêng cho bệnh nhân chẩn đoán UTDD lan tỏa di truyền có khơng có đột biến gen CDH1 Nhìn chung, với UTDD lan tỏa hiệu điều trị thấp dù áp dụng điều trị phẫu thuật kèm hóa chất bổ trợ Cắt dày tồn phương pháp khuyến cáo nhằm dự phòng khả mắc ung thư dày tương lai Tuy số điều chưa thống nghiên cứu xác định phải áp dụng điều trị dự phòng thực mà khơng cần kết nội soi Kết điều trị tích cực với nguy tử vong 1% Nhưng bệnh 49 nhân gặp biến chứng sau phẫu thuật đau bụng sau ăn, đầy bụng, khó tiêu, không dung nạp lactose, , chứng hôi miệng, hấp thu chất béorối loạn tiêu hóa Tư vấn di truyền hoạt động cần thiết cho bệnh nhân với gia đìnhcủa bệnh nhân Các gia đình có người bị đột biến gen CDH1 cần phân tích phả hệ ba hệ làm mơ bệnh học để chẩn đốn ung thư dày lan tỏa Trong buổi tư vấn, vấn đề cần tập trung, là: hiểu nguy mắc ung thư, vấn đề thực tiễn (bảo hiểm, cơng việc…), vấn đề gia đình (giao tiếp, trách nhiệm), vấn đề với (lo lắng nguy mắc con…), chung sống với ung thư vấn đề tâm lý (trầm cảm, lo âu, tức giận, cảm giác mát…) [6] Tóm lại, có nhiều bước tiến phát hiện, dự phòng điều trị bệnh HDGC nói chung CDH1 nói riêng, nhiều vấn đề thiết yếu cần nghiên cứu để hoàn thiện hiểu biết vấn đề TÀI LIỆU THAM KHẢO Bray, F., et al.,       • Epigentic có thể hiểu đơn giản là “những thay đổi kiểu hình di truyền ổn định ở chromosome mà không do thay đổi ở chuỗi DNA” [77]. Bình thường, epigenetic có khả năng methyl hóa DNA, thay đổi sau dịch mã, tái cấu trúc nucleosome, điều hòa RNA không mã hóa…[78, 79]. Theo đó, những người có biến đổi ở hệ này làm thay đổi biểu hiện của gen và mất cân bằng hệ gen. Sự methyl hóa vùng promoter của gen có thể bật hoạt gen ức chế sinh u, tạo điều kiện để khối u phát triển.

      • Một trong những phương pháp đánh giá tác động của hệ epigenetic là bisulfite sequencing. Mức độ methyl hóa của đoạn DNA được dựa trên mức độ methyl hóa của các cytosine. Mức độ methyl hóa càng nhiều thì gen càng dễ bị bất hoạt. Một số nghiên cứu ghi nhận sự bất thường của hệ này làm bất hoạt dòng mầm CDH1 hoặc đóng vai trò cơ chế bất hoạt thứ 2, gây ra ung thư [57, 80, 81]
